Iowa is full of people who make outstanding contributions to those around them every day. You may contact my office to request a letter commemorating an important milestone or outstanding achievement. These citations may be requested for:

* Birthday greetings
* Wedding Anniversaries
* Retirements
* Special occasion or outstanding achievement

To request a commemorative letter, please fill out the following form:

Please submit your request a minimum of four weeks in advance of event. There is no need to call the office to check the status of your request. You will be contacted if there is question or problem with your request.
